TAWAU O TAWAU

Dah 2 bulan aku tak update blog ni sejak kat tawau. bukan sibuk pun, banyak masa dan duit aku habis kat cyber cafe.mungkin malas nak menaip. tujuan aku kat cyber cafe adalah untuk mendengar radio light and easy radio online. tension betul aku kat sini tak dak siaran light and easy.

dah hampir 3 bulan aku kat tawau. sekarang aku senior HO kat wad orthopaedik. namanya jer senior HO, tapi macam2 aku tak tau. keje kat sini OK. tak busy sangat. sekarang lagi tak busy sebab ada 2 HO. Gajipun OK. baru dapat gaji 2 minngu lepas. seronok sakan aku wei.

sekarang dah masuk 5 ramadan. yg seronoknya berpuasa kat tawau ni waktu berbuka cepat. pukul 6 dah "sungkai" bahasa tawau utk berbuka puasa. memang tak rasa penat.

bulan lepas aku konvo kat upm. konvo tak bestla tapi boring. ambil scroll dari procancelor, bukan dari tuanku chancelor. then, tak semua family member aku datang. lepas tu, hujan pulak tue, tak sempat nak bergambar. tapi yg seronotnya aku dapat 2 hadiah: BEST O&G and best BUMI. dapatla RM1500, dan nama aku naik kat board. tersengih2 aku masa ambil hadiah.

life kat tawau ni biasa jer,, pagi dan petang keje, malam tido. tak tido pegi cyber cafe. sekarang aku tinggal kat rumah depan Masjid Raya, jalan Utara, 15 mins jalan kaki dari hospital. rumah ok cuma jauh sikit. tinggal berlima, 3 member polis marin dan 1 pegawai mara. aku sekarang ni tgh cari bilik sewa dekat kawasan hospital. rumah sekarang ni jauhla.

orang2 kat ni pelik2la. bahasa pun pelik. kalau kat semenanjung pantat adalah vagina tapi kat sini it's buttock (really strange!). kat pun ramai mamat dan minah indons. mana youngsters of tawau?

Sufiah: Since the age of 7, err…I think!



Joe: What are your other hobbies?

Sufiah: I go to the gym a lot. I love cycling and swimming. When I’m driving l love listening to ‘Light & Easy’.



Joe: Why did enter Malaysian Idol?

Sufiah: Because I’m confident and I want to compete in the global market. Plus make Malaysia proud! Hahaha!



Joe: If you don’t make it on Malaysian Idol, what else will you do?

Sufiah: I will continue working with my family. But if I am given the chance to be a recording artiste, I would take it and leave my job. I’m happy with my job but singing has always been my passion.



Joe: Describe your singing style?

Sufiah: Verys 80s like Chaka Khan or Aretha Franklin, a bit of gospel and soul.



Joe: Favorite record at the moment?

Sufiah: Jaguar Wright. Don’t remember the album name. Ok…I remember now…’Denials & Delusions’.

 

Joe: Music you would never listen to?

Sufiah: Rock, metal, hard core, death metal…lagu Feng Tau. Kalau paham bolehlah cuba.

Sufiah: I love being part of Malaysian Idol. I’m so very proud of myself. At the same time it is tiring, nerve wrecking and there is a lot of waiting. But above all that, it is exciting.



Joe: What do you think of the judges?

Sufiah: I believe everyone of the judges are entitled to their own opinion. I’m sure they have their own criteria. Well, they are just doing their job…right??



Joe: Advice for other Malaysian Idol hopefuls?

Sufiah: Take care of their well-being. Eat lots of vitamins and no late nights. If there’s spare time, conserve your voice. Sleep early if possible.



Joe: What would becoming Malaysian Idol mean to you?

Sufiah: It means the world to me. It will mean a dream come true. I promise to make Malaysia proud.

FAREWELL SUFIAH



I'm feel so sad as my future idol was not been selected for final. her name is Sufiah. During the last friday competition. she sang very well, i think better than most americant idols. even all the judges said she is a damn good singer. but things doesn't goes well for her as she wasn't selected by malaysian. i should vote for her. but the the major problem is that the mean of voting are either sending sms or dialling 600.. it cost 0.70 per sms. ai yoo mahalni. as usual i pun tengah pokai dan tak reload my hp.

Hopefully she will be selected among the 3 contestants who will be picked up by the judges for finals



 

TAWAU I'M HERE!

Received a shocking news today. I'm going to tawau for my HO posting. I put Tawau as my 3rd choice, tak sangka dapatla pulak. dengar cerita tawau tak busy sangat, so i will be happy there. Hospital tawau pun takla besar sangat. 300 bed dengan 6 specialists. macam kat kajangla. hopefully things goes well and i got my salary as soon as possible to buy the flight ticket.

  malaysian idol is going HOT. to nite 11 contestents will show their talent. i'm waiting to see my future idol: Victor and andrew. But the sad thing is that i need to use the sms just to vote them. ai yoo tengah pokai nie. my hand phone tak top up lagi.

 

 

 

PONING KOPALO DEN!

pening sungguh buat research project ni. sakit mata baca patient punya case dengan tulisan doktor macam cakar ayam. yg sekarang menjadi kerisauan macam maklumat tak dapat dicari...so dalam proforma tu isi UNKNOWN jela. ni yang payah ni. susah kalau nak analise data nanti.

Anyway the reseaarch dalam progress. setakat nie dah dapt case dekat 200. okla dapatla mengikut perancangan. hopefully dapat habis semua case by the end of next week.

SLEEP TOXICITY AND TV OVERDOSE



Semenjak cuti lepas exam ni, hidup aku memang tak terurus. tidur pukul 12 bangun pukul sepuluh. lepas tu tidur lagi pada pukul 3 hingga 5 petang. malam pula tengok tv. siang sebelum tidur tengok CD. banyak CD yg aku tengok: Bringin out the dead (cerita pasal paramedic), Scarry movies 2 and 3 (tak scarry langsung tapi kelakar habis), Shrek 2 (biasa saja) etc.

Sekarang ni aku tengah bosan. yeala buat benda yg sama. tv tue pun dah boring tengok aku. minggu lepas sepupu aku kahwin , adala tolong sikit2. penat gak angkut pinggan mangkut.

Rezeki tak tersangka datang bergolek. dr Latifah telepon aku minggu lepas dan dia nak orang tolong buat projek dia. dia bayar tau. Tah macam mana tah aku boleh tolak tawaran dia sebab aku cakap susah sangat. nasib baik dia insist juag dan dia bantu aku buat projek tu. dia bagi pinjam laptop, belanja makan etc kemudahan lain. yg penting dpt duit. aku tengah pokai ni.

mula next week. tapi sebelum tue kena buat research dulu. nak buat proforma dan testing proforma lak.

I PASS MY EXAM



Thank God! I pass my final exam. What a relief. Results were announced last Saturday. All my housemate pass except Zul. He failed Surgery and Medicine. This the mortality rate was really shocking! 17 students failed. about 20% failed in the UPM final examinations. this breaks the record in the faculty and in the country! UM mortality was 2%.

Today, went to met lecturers at faculty. met Prof Yunus, Prof Dev and Mr Kwan. They gave an advice how to become a life as a HO. life just begin man!

VIVA LISTS ARE COMING OUT

baru habis exam tadi. exam medicine. long case OK. dpt case COAD. examiner Prof Mustafa Embong dari IMU. discuss sampai management. short case not too good. 1st case was OK, case of mitral regurgitation. second case was bad. dpt case pleural effusion. dpt finding tapi presenting tak bagus. miss an obvious scar at left chest.

exam surgery not too bad. long case was ok. dpt prof yunus and prof james as my examiner. case discussed was familial adenoposis coli (FAP). short case was bad. 1st case dpt case parotid tumor (very huge one) associated with CN 7 palsy. 2nd case dpt case infected surgical wound over the medial right foot. it thought it was an ulcer, so describelah macam ulcer, tapi sebenarnya it was a surgicically created wound.

My ortho was bad. long case dapat AVN of the hip. gelabah sungguh! nasib baik dpt examiner yg OK. walau dah habis masa still discuss investigation dan management. short case SO-SO. 1st case was ganglion of the left dorsum. i got the diagnosis right but can't answer their super power question after that.

2nd case was PCL tear and medial collaterl ligament laxity. not to bad, dpt the findings tapi mistake buat McMurray's test.



nanti result viva utk besok nak keluar. viva utk distintion or extintion!

just finished my ortho paper today, not to bad. got hints from the lecturers. they asked about management of osteoarthritis, Rheumatoid arthtis, septic arthritis etc. the theory paper just finished today. next week is the clinical exams, start with surgery followed by orthopaedic and finally medicine. the clinicals are the most real thing of this exams. if I managed get thru' , I'll be doc soon, or otherwise has to study for the next 6 months.

PALPITATION, DYSPNOEA AND REDUCED EFFORT TOLERANCE



lagi 16 hari nak big bang exam, final exam of the year. the exam where i can be a doc or not. takut siut. sekarang dah ada palpitation. makan pun tak lalu tapi tidur over-over lak.



pagi tadi baru jer buat short case kat wad P2. banyak jugak case respiratory, cardiovascular, pns etc.

aku rasa banyak aku tak tau. especially pasal fundoscopy dan orthopaedic. surgery macam ok tapi dah banyak berkarat. medicine lagilah, makin banyak baca makin banyak tak tau.

entahlah, i really hope that i can go through this exam. tak larat nak ikat perut sebab tak cukup duit. yea la dah la sewa rumah mahal, parents aku pun tak dapat tolong.

kalau dah kerje, i have my own money dan boleh lah survive kat malaysia ni.

aku appply ho kat hosp klang, melaka dan tawau. hajat dihati nak letak tawau pilihan no 1. tapi tak der duitla.

TENSION



baru habis exam mcq ortho, susah nak mampus, 10 soalan jer tapi carry 100%. aku kena 5 soalan betul nak pass..rasa macam tak pass jer, soalan tricky.

actually bukan study banyak mane pun, lepak sakan.

hari nie lagi satu tension, exam clinical sepatutnya pagi tadi, tapi lecturer mintak postponed hari jumaat. penat tunggu selama 2 jam kat wad 2 (ortho wad). budak lain dah habis exam clinical. nora dpt case klumpke's palsy. tse nee dpt case vertebral infection dan hema dpt case TB of spine. kesian bebudak ngan prof devnani pagi tadi. habis kena bom. dia tanya pasal antibiotic eq metronidazole, penicillin. sorang pun tak dapat jawab. Kalau aku ngan prof devnani mesti aku tension jugak

I FEEL GOOD



The whole last week i was at the National sport institute, KL. what a great place. here is the place where they trained professional atheletes. The posting started on Tuesday (24/02/04) [mon is a public holiday] . arrived there at 9.00 am, we used a LRT b'coz the faculty does not provides a bus that day. welcomed by a cute and charming AP dr lim, he brief us what to do here. later, his assistant, ms sue git bring us around the place (tourlah). nice place and great facillities. they have a pool, gym, exercise physiology, counselling room etc.

at lunch time, we played basketball. with a tie a casula shoes i shut 3 balls. the game was great. this is a 1st time we played together within 5 years. actually we played like a net ball as most of the girls were net ball players. in the afternoon, was spent at the exercise physiology room. met mr patrick (canadian), he thought us how they assess the fitness of an athelete. limi offered to undergo the process. in a machine, with a running floor, she runs at the gradually increasing constant speed. her mouth was connected to a com where her O2 consumption and CO2 product can be measured. she got tired at stage 2 (ok lah for girls).

here we had a lot of activities.

at morning we joined sport clinics. adalah tengok kes sport injuries.

in the afternoon, we went to the pool and learned about aqua jogging. this help in rehab pt. met ms sandra, a tall mat salleh girl. we also play a games with the pt in the pool. we lost 0 to 9 points. the next day, met her again and she thought us about exercise in a gym.

we met a few numbers of well known atheletes. took a picture with lee lam wah. met nazmizan (sprinter) who broke an asean games record. saw hafiz hashim but don't dare to approach him (seganlah) while he was in training.

the last day was ok. we had a psychology session. here we need to tell about our selves to the group. i knew a few of my friends secret, one of my friend cried (don't expect her to cry b'coz she is hunky punky gal). what a shock!

the posting here was enlightment me to a new field of medicine, where medicine is applied to sport. it is an interesting field and wish to come back there.

CANTIK SIUT!



hari nie dah 3 hari posting kat hospital selayang, posting pasal penyakit rheumatology. cantik betul hospital selayang nie, macam hospital private. sejuk ber aircon, tak macam hospital kuno kerajaan lain. wad pun luas. katil pun baru. setiap pesakit dpt cubicle sendiri beserta ngan tirai. tak macam ngan HKL. tirai pun tak de. sometime kalau nak buat per rectal examination pun tak bertirai, open!

Nilah kelebihan hospital baru. kerajaan sebaik mungkin nak perbaiki perkhidmatam kesihatan rakyat.

Workload kat sini pun tak banyak. so boleh concentrate pada patient in detail dan hopefully tak mismanaged.



KEs-kes rheumatology kat sini memang menarik. adela tengok kes SLE dan rheumatoid arthritis. pagi tadi clerk case patient suspecting ada connective tissue disease. History pt nie:

c/o bilateral calf pain for 3 months

-worse on walking

-relieved at rest, taking analgesic

-intermittent, moderate in severity



went to nearby clinic, blood Ix showed raised ESR,

referred to Kajang hospital,

xrays done: normal, discharged but developed,

bilateral ankle swelling for 1 month

sudden in onset.



also had LOW, LOA, sms of anaemia

no history of alopecia, malar rash, oral ulcers, joint pain, fit

no family history of connective tissue disease



o/e

cachexic, pale, no jaundice

left supraclavicular bruit

CVS, Resp and abdomen normal

livido reticularis both lower limbs

erythema nodusom both feet

generalized hyperreflexia



What is yr dx?





p/s they are thinking of polyarteritis nodosa, investigations are waiting.

A DAY OF SORROR.



pagi tadi melawat HUKM. cantik hospital tue, macam hotel 5 bintang. belajar mengenai alatan orthotic dan prosthetic. Mr Lim (owner of Limb brace sdn bhd) ajar tentang penggunaan prosthetic dari preop sampailah pemasangan prosthesis. amputated stump yg baik mempunyai wound yg sembuh sepenuhnya dan tiada oedema. pesakit amputee selalunya mempunyai kesan sampingan "phantom limb". pesakit ni merasai kesan sakit pada kaki yg tiada.

lepas melawat HUKM, pulang semula ke HKL.

Petang tadi melawat ward spinal. Wad spinal HKL adalah yg pertama di M'sia. Dpt tengok kes spinal termasuk tetraplegia dan paraplegia due to accident, TB etc.

Sister OM ajar macam mana utk prevent pressure sore. all patient who are plegic need to be turning 2 hourly eg from right lateral to left lateral. bahagian kaki dan bahagian belakang pinggang disupport dengan bantal.

Kalau mampu boleh guna Ripple mattress.

Sempatla jugak sembang dengan En Khairuddin, a complete tetraplegic. He suffered from tetraplegic after had motor vehicle accident 8 years ago. dia dtg pasal extensive pressure sores over the sacroiliac region.

kesian tengok abang - abang paralysed kat sini. one of the pt cry in front us. we won't bother to ask why. I can't imagine how they can cope by lying down almost of the rest of their life.

I think most of the patient here have a good will. they really want to survive and get better. May Allah be with them always.

here they will undergo extensive rehabilitation program for a minimum of 6 months. if OK, they will sent back home or go to the Pusat Latihan Perindustrian Bangi.

last friday wuz great! I dapat salam ngan the Agong! jgn mare u! 2 x lagi. dia sajer melawat HKL dan masa sama semayang jumaat sekali. this remind me with the previous Agong: Sultan Salahuddin Abdul Aziz Shah, dia pun I dapat salam. masa tue dia melawat Masjid UPM kat serdang dan semayang jumaat sekali.

today activities were entirely spent on rehabilitation medicine. i went to the occupational therapy department and was been thought about their activities there. quite interesting, macam - macam alat boleh digunakan uth rehab perpose. This morning discussed case of spinal injury and the rehab aspect. Arisah yg present. okla, sekarang baru clear pasal management nih.

besok pergi melawat HUKM . nak tengok pasal prosthesis and orthosis.

pagi nie bangun lambat, 8.30 am. tak der kelas, cuma ader klinik orthopedik ngan My Hisham (my tutor). tengok kes # left femur (operated), spine # of L2, "motherland" disease dan a man dtg ngan pain and swelling at right wrist. okla, ader lah blaja. petang plak ade seminar pasal limping child. budak2 yq present, conduct by Mr Hisham. yg penting topic congenital dysplasia of the hip: mesti tahu ortholani dan berlow test, SUFE (slipped upper femoral epiphysis), congenital talipes equinus varus (CTEV) dan Perthe's disease. CTEV penyakit budak kecil, kaki die terpelengot (eja?) kedalam.

Tadi Seng Chai bagi ang pau (best nyer), RM 5 , boleh shopping. dia ucap thanx kat aku sebab selalu call dia kalau ader kelas. budak2 chinese lain bagi limau dan kuih kapit. yea dpt 4 biji limau (i love oranges).

ade sesiapa baca surat khabar hari nie? pembunuh adik nurul huda dihukum penjara 20 thn dan 24 sebatan. he deserved it selepas ape yg dia buat pd adik nurul. lepas ni die kena bicara kanun keseksaan 302 yg boleh dihukum gantung sampai mati.

hari nie dah masuk minggu ke3 posting orthopaedic (pasal tulang). nielah posting paling blur , dah masuk 3rd week masih blur2. lecturer tanya plak senyapjer sebab banyak tak tau. pagi tadi ade grand ward round ngan Mr Ariff. kejap jer, tak sampai pun sejam. dia pun macam tak bersemangat pasal yg dtg cuma 3 org jer, yg lain dah mula cuti thn baru cina. dia tanya pasal diabetic foot: classification dan about Rays amputation. macam biasa aku pun tak tau. apalagi dia pun laser lah " you all nie dah masuk 3rd week pun tak tau commom cases in the ward, diabetic foot is very common and don't take your exam if you don't about this!, I nak you buat assignment pasal diabetic foot dan present next monday".

posting ortho nie banyak lecturer yg garang2 terutama Prof Devnani. last week dia tengking aku masa grand ward round. salah aku jugak sebab aku tak prepare utk present case.

Aku: The patient is 17 year old male presented with 1 day history of backpain

Prof Dev: Not presented but complained of........

Aku: Patient has a 5 year history of bakpain..

Prof Dev: WHAT????? why suddenly u say 5 year history but the patient presented yesterday?

Masa nie aku dah confuse, pt cakap dia ade intermittent backpain for 5 years dan the backpain become suddenly worse yesterday. Prof Dev cakap I buat dia confused dan history aku masih tak lengkap. he wish me luck for the becoming final exam.

Kena lagi aku.

Satu lagi lecturer yg garang ialah Miss Zanariah, dia cukup fed up kalau student tak tau anatomy.

Exam final MD lagi 3 bulan, aku dah rasa nervous. banyak Woo kena baca. walaupun banyak kena baca tapi aku masih lepak2. semlm tengok tv sakan: senario dan national geographic.

pagi nie dah lepak2 kat lib surf internet yg mengarut.

Sejak masuk final year nie rasa macam dah malas, sebab? aku pun tak tau. mungkin dah saturated kot. tapi org cakap kite hanya pakai 11% of our brain capacity. or my brain has little capacity? tapi boleh masuk med school!

hari nie last posting senior medicine, tadi baru jer habis class terakhir ngan prof ng dan dr loong. bincang pasal exam minngu lepas. aku pass, soalan ok jer. lepas class ader jamuan harijadi arisah. makan cake, makan ngan prog ng dan dr loong. sedihla pulak tinggal medicine..inilah perasaan aku each time habis posting..yela nie lah kali terakhir aku belajar medicine as a student. lepas nie jadi dr dan banyak responsibility. ilmu masih cetek dan pengalaman pun tak meluas. lepas nie mesti aku rindu kan prof ng..he is really funny guy, but when time to work he is really serious!. what is the cause samine!!!!!!hah hah hah !!!!
