Mr Prakash

Terima kasih Bos!!!.

When wearing tie is dangerous

Koh and his associates have published a very interesting research article in recent Medical Journal of Malaysia. He conducted a research about doctors neckties whether it's contains the super bug/germ ie the MRSA (Methicillin Resistant Staphylococcus Aureus).
MRSA is a super bacteria that has capability against the standard antibiotics. We feared most of this bacteria. Patients who contacted MRSA will die and they needs special antibiotics for the treatment. (to kill the bacteria).
To our surprised, Koh and his associates discovered that more than half (52%) of neckties worn by doctors were contaminated with Staphylococus. Out of these, 62% of them were identified as MRSA!!!.
We took this report seriously. Doctors in my department are no longer wearing neckties. Infact, in the near future we are not allowed to wear any hand watches, long sleeve shirts , jewelery and white coats.
This is an important step to reduce the infections.

(Nota kaki: mula-mula dah tak bole pakai tie, kemudian tak bole pakai baju lengan panjang, jam tangan dan cincin...nanti pergi keja tak payah pakai apa-apa..he he he)

Tugas Mesti Kena Buat

Dedicated this puisi to Puan Pengarah, my bosses, kawan-kawan, nurses and others.

tugas mesti kena buat,
walaupun hospital kekurangan ubat,

walaupun hospital kekurangan anggota perawat,
walaupun hospital kekurangan alat-alat,
walaupun alat-alat itu sudah berkarat,
walaupun alat-alat itu penuh berulat,

tugas mesti kena buat,
kita mesti pulang lewat,

kita tak boleh rehat,

kita tak diajar erti penat,
kita tak diajar untuk mengumpat,
kita tak diajar untuk pulang jam empat,

tugas mesti kena buat,
tugas kita memang berat,
kita sudah diberi amanat,
untuk merawat pesakit-pesakit tenat,

supaya mereka sihat,

dan kembali ke masyarakat,

tugas mesti kena buat,

hatta dilabel seorang penjahat,

hatta dilabel seorang pengkhianat,
kita adalah orang hebat-hebat,
kita mempunyai tangan keramat,
lebih baik dari orang berpangkat,
lebih baik dari yang berhormat,

tugas mesti kena buat,
buatlah cepat-cepat,
pastikanlah selamat,
jangan lambat-lambat,
takut nanti tak sempat,
dan jangan lupa berjimat cermat,

tugas mesti kena buat,
walaupun tiada keluarga terdekat,
selalulah beribadat,
solat, selawat, puasa sunat,
supaya mendapat berkat,
supaya tak kena laknat,

tugas mesti kena buat,
meskipun esok kiamat,
meskipun ajal dijemput malaikat,
meskipun didalam liang lahat,
supaya masa di akhirat,

kita tak kena SEBAT!!!

Ehh..buat keja la,, jeling-jeling plak.

Rhymes

A rhyme is a repetition of similar sounds in two or more words and is most often used in poetry and songs.
Generally, rhyme can refer to various kinds of phonetic similarity between words, and to the use of such similar-sounding words in organizing verse. Rhymes are classified according to the degree and manner of the phonetic similarity:

  • syllabic: a rhyme in which the last syllable of each word sounds the same but does not necessarily contain vowels. (cleaver, silver, or pitter, patter)
  • imperfect: a rhyme between a stressed and an unstressed syllable. (wing, caring)
  • semirhyme: a rhyme with an extra syllable on one word. (bend, ending)
  • oblique (or slant/forced): a rhyme with an imperfect match in sound. (green, fiend; one, thumb)
  • assonance: matching vowels. (shake, hate) Assonance is sometimes used to refer to slant rhymes.
  • consonance: matching consonants. (rabies, robbers)
  • half rhyme (or sprung rhyme): matching final consonants. (bent, ant)
  • alliteration (or head rhyme): matching initial consonants. (short, ship)

ok..let try rhymes.. this is my first attempt..

Received a CALL,
It's a nurse from NEPAL,
I was at the MALL,
She said patient had FALL,
His name is Mr PAUL,
He hit his EYEBALL,
Luckily his wound is SMALL.

Now, lets try in Malay language..

Jam sudah 2 PAGI,
Nurse telefon doktor JOHARI,
Doktor Johari perlu pegi OT,
kerana ada OPERASI,
Semua orang dibuai MIMPI,
Tapi doktor Johari BUSY,
Perlu buat APPENDISEKTOMI,
dibantu oleh Doktor MIMI,
operasi siap jam 4 PAGI,
doktor johari tinggal kan OT,
berjalan KAKI, ke kedai dan minum KOPI..

OT = operation theater
Macam mana? Ok tak..

The Wishing Tree...and Seri Mengasih Centre

SERI MENGASIH CENTRE

P.W.D. 75, Jalan Selangor, Tanjung Aru, Kota Kinabalu, Sabah. Tel: 088-223 221; serimengasih@yahoo.com.my / pruian@tm.net.my

This is a special developmental centre providing a comprehensive range of training programmes for the intellectually disabled and services for their families. It was established in 1981 as the first special school in Sabah for children whose special requirements could not be met in mainstream schools.

The project was initiated by a group of volunteers in collaboration with Sabah Mental Health Association and established by a board of management.

A footballers’ changing room at the Likas Sports Complex was used to start the first class for 10 children aged six to 12 years. The pioneer staff consisted of three teachers, a general helper, and a Cuso (Canada) volunteer as its first principal.

In 1983, the school shifted to a government residential building with a spacious compound in a choice area by Tanjung Aru beach in Kota Kinabalu. After a span of 20 years, Seri Mengasih now has an enrolment of more than 150 students, ranging from babies to young adults, and 25 staff members.

The school seeks to raise the hope and dignity of the intellectually disabled by providing for their holistic development from the earliest age until, as far as possible, they have achieved socio-economic independence and are successfully integrated into the mainstream of community life.

This is done through the combined effort of the staff, parents and the general public.

Wishing for:

·A self-contained square mobile booth to serve as a sales outlet for products made by vocational trainees.

·A house or apartment near a bus stop to train adult students to live independently within the community.

·A van to transport students and teachers.

·Heavy-duty plastic chairs.

·Stand fans.

·DVD players as teaching aids.

Escorting patient

It was Sunday, 29th November 2009. I was not oncall/working. I was lepaking in my room, half naked enjoying the weekend while listening to Elvis Presley CD. I just bought this CD a day earlier.

Then, i received an unexpected call from my colleague.

DrF: Bond..ada 1 baby with gastroschisis la..perlu hantar ke Kota Kinabalu..
Me: hantar la g KK..
DrF: perlu doctor escort la..baby tak cukup bulan..berat 1.9kg..DrC and DrO oncall hari ni..aku oncall esok..DrR cuti..bos pun cuti..ko jer yang free..
Me:..ala..tak kan nak gi KK..aku baru jer balik dari KK..
DrF: ala boleh la..
Me:ok la ok la..semua dah arrange ker??
DrF: DAH!. Nanti driver sampai pukul 11am.

That's it!!! my weekend is spoiled. I have to go to KK today escorting patient.
I got up..and get dressed and start packing.
By 11am, i was at the hospital and straight away to the nursery ward.
Nursery ward.
There, i met a cute and small little girl. She has gastroschisis.
The girl.
Gastroschisis is condition where the baby was born with a defect over her abdomen (tummy). When there is a defect, there is a hole where the bowels can come out. It is a surgical emergency and this patient needs urgent transfer for immediate operation.

Looking at the patient case note, i was surprised that she was admitted last night. Now it's almost 11am, the arrangement for the transfer still not yet complete.
The arrangement is important. Before leaving the hospital, we have to make sure that our equipment, medication, oxygen , xrays, letters etc etc are complete. We don't want when we are in the middle of Borneo jungle..then we just realized that the oxygen tank is empty!!!. Thats will be disastrous and unfortunately it does happened before.
One of my colleague escorted a patient to KK and she forgot to bring the patient's xray. After arrived in KK, she got very "nice" remarks from the doctor there.
The ambulance.
Getting ready. The oxygen tank is empty. I asked the driver to get a new one.
About to leave..
After everything was arranged, we left Sandakan Hospital at 1.30pm.
The captain..Mr Miki.
During the journey, this baby is at risk of developing these conditions:
  • hypoglycaemia...blood sugar become low..
  • hypothermia..baby temperature goes down..
  • apnoea..baby can't breathe properly and her oxygen concentration in the blood goes down.
These 3 conditions can be fatal. Hence, during the tranfer she was keep inside a incubator/container. This incubator has temperature monitoring and we can adjust the temperature accordingly.
Baby inside her "space station".
She also was connected to continuos pulse rate and saturation monitoring.
When she can't breathe, the saturation monitoring will goes down..alerting us to do something quick.
The monitor.
She was closely monitored during the entire journey and all her clinical data were recorded.
Baby is sleeping.
The mama and my nurse.
As expected, the journey was bumpy. The road in Sabah was not like in the "west". The road is small, and "berbelit-belit".
We were passing jungles, kampongs , rivers and hills.
Enroute to Telupid.
A river, enroute to Ranau.
A river.
Before reaching Ranau.
It's jungle out there!!
Ranau..a kampong
Leaving ?Ranau..I forgot where is this place
Kundasang.
Kundasang
Kundasang
We just left Kundasang, going to Tamparuli,,then i saw this..

It's a rainbow!!
Despite it was raining almost during the entire journey, we reached KK safely at 6.30pm.
Almost there..this is Tamparuli.
We headed to Likas Hospital and transferred this baby there.
Lets go.
Finally..
Good bye baby..be a good girl.
We left the hospital by 7pm and immediately i asked the driver to go to the airport. I wanted to catch a last flight to Sandakan...but it was too late. Instead i was given a flight to Sandakan the next day..I have to go back as soon as possible, as the next is Monday, and the hospital will be busy.
Overnight, i stayed in a hotel. I was really tired but didn't managed to get a nice sleep.

The following day, i was at the airport by 6am and the flight to Sandakan departed at 7.10am.
Meanwhile, my nurse and the driver started their journey back to Sandakan by an ambulance.
The KK airport.
I was in Sandakan by 7.50am and i was picked up by the hospital driver back to the hospital where i continue working.
