5 days to go


Home sweet home.

Hati ini sungguh girang,
Menanti 5 hari mendatang,
Untuk pulang ke Jinjang,
Bertemu orang yang kusayang.

In a just few weeks to come, that apartment will be mine.
Has been waiting this moment for 5 years.
I will be busy next week, signing up the documents.

Tok Mudim



Tok Mudim is a malay word for a person who performed circumcision.
Circumcision is the removal of some or all of the foreskin (prepuce) from the penis. The word "circumcision" comes from Latin circum (meaning "around") and cædere (meaning "to cut").

According to the World Health Organization (WHO), global estimates suggest that 30% of males are circumcised, of whom 68% are Muslim. The prevalence of circumcision varies mostly with religious affiliation, and sometimes culture. Most circumcisions are performed during adolescence for cultural or religious reasons; in some countries they are more commonly performed during infancy.

There are many ways of doing circumcision. I prefer the "dorsal slit" technique as it's the most safest.

Now, lets have a look at this technique..

This is the dorsal slit technique. The foreskin is cut at the dorsal part (bahagian atas penis).

It's took me about an hour to complete the procedure...a bit slow from the experts. I'm doing slow as i just want to make sure that this boy do not end up any complications later.

Bersama Puan Pengarah

It was usual busy Monday clinic. I was attending patient, then i received a surprised call from my Puan Pengarah.

Pengarah: Bond, come to my office now, i have something to tell you.
Me: ok, ok..

Straight away i went to her room.

Inside her room we have very ultra-short conversation.

Pengarah: Bond, please have a seat.
Me: thanx, why Pengarah want to see me?
Pengarah: ohh..i have good news. I decided to submit your name and the other senior doctor to the ministry. This is about promotion,,we are applying post for u48 (memangku).
Me: Ohh..what a surprised!! i felt honored.
Pengarah: You are very good doing your work.
Me: ermmm...is this because i won second place in national level last year??
Pengarah: sort off..

I was about to leave her room, then she continues talking.

Pengarah: BUT, before i submit your name, i want you to improve your EQ...
Me: what?? EQ??
Pengarah: Yes..EQ..meaning emotional quotitient.
I received complains from the nurses saying that you have very low EQ.
You hardly smiles, you hardly talk and you always grumpy.
Don't follow your boss. He is old. He can't be changed.
I think you can changed.. Boleh ka??

I wanted to end this conversation..this is my reply.

Me: i'll try my best.

I left her room with mixed feelings.

Gugurkan tuntutan

KOTA KINABALU 10 Jan. – Akhbar Herald-The Catholic Weekly perlu bersedia menggugurkan kes tuntutan penggunaan kalimah Allah oleh bukan Islam di mahkamah berikutan perkembangan yang semakin membimbangkan hingga mengakibatkan beberapa gereja dibakar.

Presiden Majlis Jaksa Pendamai Malaysia, Datuk Clarence Bongkos Malakun berkata, langkah tersebut perlu untuk keamanan dan kestabilan rakyat berbilang kaum di negara ini.

‘‘Sudah tiba masanya kita, masyarakat Kristian khususnya Katholik membuat penilaian kepada keadaan sekarang demi keselamatan dan ketenteraman awam supaya dengan senang hati menggugurkan perkataan Allah untuk menenangkan saudara Islam kita.

‘‘Daripada terus mendesak hak untuk menggunakan perkataan Allah adalah lebih baik jika Katholik hanya menggunakan perkataan ‘Tuhan’ atau ‘Tuhan Yang Maha Kuasa’.

‘‘Biarlah saudara Islam kita sahaja yang menggunakan perkataan Allah untuk kebajikan supaya kita dapat hidup aman damai dan harmoni di Malaysia,” katanya dalam satu kenyataan di sini hari ini.

Kelmarin, dua gereja – satu di Desa Melawati, Kuala Lumpur dan satu lagi di Jalan Templer, Petaling Jaya – mengalami kebakaran manakala satu lagi juga di Petaling Jaya menerima ugutan.

Kejadian itu mendapat kecaman daripada parti komponen Barisan Nasional (BN) dan pembangkang selain turut mengundang reaksi serupa dari pertubuhan bukan kerajaan (NGO).

Perdana Menteri, Datuk Seri Najib Tun Razak yang melawat gereja di Desa Melawati, semalam, mengumumkan peruntukan RM500,000 untuk membolehkan pentadbiran gereja itu membuka rumah ibadat baru di kawasan lain.

Sementara itu Clarence berkata, alasan sesetengah pemimpin Kristian bahawa kalimah Allah perlu dibenarkan hanya kerana ia sudah digunakan sejak sekian lama adalah tidak wajar.

Tegas beliau, Kristian di negara ini juga tidak sama seperti diamalkan di Indonesia, Mesir, Iraq atau Yemen kerana Malaysia mempunyai perlembagaan yang berbeza.

‘‘Tidak harus bagi kita mempertikai yang Allah biasa digunakan oleh masyarakat Kristian di negara luar kerana kita berada di Malaysia, jadi kita harus patuhi semua undang-undang dan peraturan.

‘‘Adakah kalau kita atau pihak gereja tidak gunakan perkataan Allah maka secara tiba-tiba para pengikut agama Kristian akan bertambah atau berkurang 10 kali ganda,” soalnya.

Beliau sedar ramai penganut Kristian akan mengkritik pendiriannya tetapi mereka perlu faham dan menghayati ajaran agama itu yang menyuruh pengikutnya memaafkan orang lain.

“Jika kita benar-benar Kristian, kita harus jadi orang yang memaafkan, bertanggungjawab dan cintakan keamanan. Kenapa kita harus bergaduh untuk satu perkataan,” ujarnya.

Clarence yang juga Pengerusi Institut Kajian Pembangunan (IDS) Sabah berkata, umat Kristian perlu faham mengapa orang Islam amat marah apabila kalimah itu dibenarkan penggunaannya.

Jelasnya, Allah adalah Tuhan bagi orang Islam manakala orang-orang Kristian pula lain kepercayaannya dan kalimah itu juga sudah lama digunakan secara tradisinya oleh agama Islam.

Hypocrites



I always get angry when my patients didn't take their medications,
But when i got sick, i didn't take my medication too.

I really upset (especially my brother) if he still smoking,
I always tell him, smoking is bad for health.
But i do smokes, occasionally.

I always tell people to pray,
But sometimes i didn't pray,

I always told my patient to do exercise,
It's good for health.
But i hardly do exercise.

I always told my patient..relax!! don't be stress up..
But i'm the one who always stress up.


This is an example of a hypocrites. In Malays it is called "talam dua muka".
People said hypocrites people die faster and go to hell.

Puisi ini ditujukan kepada semua penjaga-penjaga pesakit di hospital seluruh Malaysia. Tak kisah la, sama ada isteri jaga suami, suami jaga isteri, anak-anak jaga ibu bapa, abang jaga adik, adik jaga abang etc etc. Penjaga-penjaga ini memerlukan kesabaran yang tinggi kerana menjaga orang yang sakit memerlukan pengorbanan yang besar. Should try this centuries ago..

Despite fever has resolved, i'm still having sore throat and cough.

Has been trying strepsils..dah berkotak-kotak try..still not working. Still has painful throat and coughing.

Two days ago, someone tried to convince me.

Bond!! why don't you try Diflam mouth wash. I tried it before and it does work well for me.

He said it twice but I didn't pay attention. I didn't listen to him. I'm still thinking strepsils still work.

Today, the sore throat is getting worse.
Finally i decided to try the Diflam mouth wash.
Went to my hospital pharmacy and they supplied 2 bottles of Diflam mouth wash.

Diflam mouth wash: the green colour.

I went home, i just tried a sips of Diflam mounth wash.

Amazing!!! the sore throat and coughing much less...

Jalan - jalan di Taman Jalil Alip

Sorry for delayed entry..
it's took me about a week to download those pictures from my hotmail inbox.

It was Sunday 3rd January 2010. I was (almost) recovered from my illness. Then, around 3.00pm, i received a surprised call from Mr Iqmedia..and he already infront of my house..

"Get dressed..lets go jalan-jalan.."

What? pergi mana?..i replied.

"Adalah"

I dressed up.and here goes the trip.

We went to Taman Jalil Alip, Sandakan.
This park was developed by Mr Jalil Alip, a local businessmen.
I never heard this park before. I have met many tourists and i don't think they know about this place.

The journey took about 45mins. It was quite remote. But going there was easy as the road is well developed.
It's costs RM 2 per entry for an adult.

My first impression was this park look like it's was deserted. But it's still well maintained and clean.
It was 4.00pm that day. Less than 10 people were at the park, very unusual as today is public holiday.

(Nota kaki: with much publicity, i think this park can become another tourist Sandakan attraction. It's really nice place for family activities, people who want to do outdoor activities, nature lovers or "kaki pancing". I saw at least 3 big ponds there!!. This place is cosy and it's surrounding is serene.)

It was usual busy Monday surgical clinic. I was called from my boss to his room.

Boss: Bond..please come to my room now, there is special case i want to show to you.
Me: ok ..ok.

When i was in his room, i saw a middle aged man lying on the examination couch.
Then, my boss asked me to examine his genitalia.

WOW..what a surprised!

His penis is larger than average man in Sandakan.. (obviously i've seen and examined many male genitalia in Sandakan).

I asked this question to this patient.

drBond: Is this natural???
He replied .."No"..

Then our conversation continues (i'm curious)
drBond: So how come it become so big. Belum pun ada erection.
Patient: errr..doctor, i pergi Filipina tahun lepas,,buat suntikan silikon..
drBond: ohh..jadi.?.dah besar! ...dah dapat apa yang nak, kenapa nak jumpa kami???
Patient: memang la besar...tapi tak naik la..lepas kena suntik batang dah tak naik!!!
drBond: ohh ya ka?? kenapa jadi macam tu??
Patient: tak tau la..
drBond: kenapa you nak buat suntikan untuk besarkan?? wife cakap kecik ker??
Patient: Tak...wife tak cakap apa-apa..
drBond: So??
Patient: sorry ya doctor...hari tu saya ada pergi mile 4, main perempuan, itu perempuan cakap batang saya kecil la....itu sebab saya buat suntikan..
drBond: ohhh..

Thats end our conversation. From that history, we have to take blood investigation like HIV, VDRL (for syphilis)
He is planned for refashioning of his penis later.
My theory is that, after his penis was injected with silicone, this material has disrupted the blood supply and blood flow of the penis.

This blood supply and the blood flow is important for erection of the penis. Hence, once it was disrupted, there is no way the penis can erect.

2. Haram menyuntik silikon dalam penis, kerana boleh mendatangkan musibah, seperti kes diatas.

Marah itu ?Indah

Hari Khamis 7 Januari 2010 adalah hari OT...(OT day).
OT bermula pukul 7.45pagi.
DrBond telah diamanahkan untuk menjaga pelajar perubatan UMS. Mereka buat attachment di HDOK, Sandakan untuk belajar subjek surgeri. Attachment di jabatan kami hanya 8 minggu.
Masa awal-awal lagi sudah diberi orientasi. Dah pesan dan sudah beri ingatan..student perlu masuk OT seawal 7.30 pagi.

Pada hari ini, dah hampir pukul 8.00pagi, seorang pelajar perubatan tidak kelihatan.
DrBond mulakan operation seperti biasa..pembedahan minor..hernia repair.

Kemudian, hampir pukul 9.00pagi, 3 orang pelajar perubatan masuk OT.

Bagaikan gunung berapi, mulalah perasaan marah membuak-buak. Seperti gunung berapi memuntahkan lava-lavanya.


DrBond: Hei!!!! Sekarang dah pukul berapa???? Kan dah pesan kena masuk OT pukul 7.30 am???

Student: senyap..

DrBond: korang buat apa dari pukul 7.30am sampai 9.00am??

Student: errr..then terus senyap..

DrBond: korang ni kenapa?? kan dah pesan masuk OT 7.30am. Bila datang awal boleh join and scrubs with me. I need assistance OK!!.. You all pun perlu isi dalam log book ..at least mesti scrub sekali.

DrBond: berenti sekejap...(tarik nafas)..This is my warning..Any student yang tak pernah scrubs..tak bole ambik exam end of surgical posting. pok pek pok pek pok ..

Sambil buat operation sambil marah medical student.
Dalam OT..nurse2 senyap ketakutan.
Dah lama DrBond tak marah-marah.

Operation siap pukul 10.00am. Kemudian, DrBond rasa sakit kepala dan pening.
Inilah akibat selepas marah. Selepas marah sakit kepala dan feel dizzy.
adoi..sakitnya.
Sakit kepala terlalu kuat, terpaksa telan 2 biji panadol.
DrBond tak boleh sambung kes OT. Terpaksa masuk bilik oncall dan tidur sekejap.
Nasib baik sakit kepala dan pening reda lepas pukul 3 petang.
Tapi sakit hati masih terasa sampai hari ni.



(Nota kaki: to all medical student...semua dah dewasa, tak payah la nak kena layan macam budak sekolah. DrBond malas nak ambil kedatangan..bila tak datang nak repot dengan dean.. Eiii Malas la..Tak kuasa nak buat semua tu. Banyak lagi kerja yang berfaedah perlu buat. Sendiri mau ingat..nak jadi doktor tak boleh main-main dan ponteng..
And DON'T WASTE MY EFFORT!!!!!)

Shame on me.

I was at the male orthopaedic ward today at 8.30pm. This is a ward that managed patient with orthopaedic problems eg bone fracture.
I was at their ward to see patients that need surgical opinion.

I was writing on the patient's case note near the nurses counter.
Then, i heard a man recite an Quranic verses.
He was lying on the bed.
He was praying...performing the Isya' prayer.
I asked the nurse..whats wrong with him.
She said, that man was just admitted. He has involved in motor vehicle accident and he has fracture of the femur. (long bone).

Fracture of the femur is very painful condition..but looking at him, he appeared calm and still able to perform his duty as a Muslim.

After that, a young man with pink T-shirt came to see me.
He has fracture of the humerus..another painful conditions.

He asked:

Doctor..where is the direction of Kiblah???

I pointed out..this way..

Then, he went to his bed and performing his Isya' prayer.
He did that on sitting position.


I was impressed by this 2 patients.
They are sick...instead they able to perform a duty as a Muslim.

Thats really put shame on me..because..now i'm consider my self healthy..but sometimes i didn't perform my duty as a Muslim.

Chemotherapy in breast cancer

A 65 year lady was presented to my clinic a year ago with left breast lump for 6 months. All the workout done and it was confirmed as cancer. She was counseled for surgery but she refused and defaulted treatment.
She came again 6 months later. Now, the left breast lump was huge.
very huge breast tumour over left breast.

She requested for surgery. But, at this moment surgery can not be done as the tumour was huge, making surgery difficult and risky.
Instead she was offered a chemotherapy.

Chemotherapy is a treatment that use anti-cancer drugs to destroy cancer cells. Women with breast cancer usually received chemotherapy in either of these forms:
  • before surgery, like in this patient, to make the tumour shrink, then patient will undergo surgery. This is called "neoadjuvant chemotherapy".
  • after surgery, and this is called "adjuvant chemotherapy.
There are many anti-cancer drugs for breast cancer available in my hospitals, among others are:

In higher centres with oncologist there is a newer drug called "targeted treatment" like Herceptin. But this treatment is expensive, patient needs to pay almost RM100, 000 to have this anti-cancer drug for a year.

Patients will received combination of anti-cancer drugs, like:

  • FEC – 5FU, epirubicin and cyclophosphamide
  • AC – doxorubicin (Adriamycin®) and cyclophosphamide
  • CMF – cyclophosphamide, methotrexate and 5FU
  • E-CMF – epirubicin and CMF
  • FEC-T – FEC plus docetaxel (Taxotere®
Usually, chemotherapy is given as injection form. In our hospital, patients needs to be admitted to have the treatment. In other bigger hospital, the treatment can be given as an outpatient. After the treatment, there is followed by a rest period of a few weeks, which allows the body to recover from any side effects of the treatment.

Side effects from chemotherapy is common. The usual side effects are:
Fortunately, the side effects are minimal and we do give patients some medication to reduce the side effects.


Back to the patient, she has completed the chemotherapy.
Alhamdulilah, the tumour shrink remarkably and she is scheduled for surgery probably next week.

See..no more tumour..amazing right????

The Value of RM 1000



What is the value of RM 1000.?
If you asked that question to a poor family, they will said that amount of money is a lot. They can have dinner for at least 2 months.
If you asked that question to a rich family, they will said it's nothing. They can spend it in less than a minutes.

But if you asked me the same question...this is my answer..

It's precious. With that money, i will having my eldest brother till today.


It was the end of October 2004. I've already worked as a houseofficer in Tawau Hospital almost 3 months. That day was a payday. I've received my salary for 3 months in a lump sum. It's almost RM10, 000.
That was my first time having this huge amount of money.

At the same time, i received a news from my mum saying that my eldest brother business is not good. He has a small shop in a local university selling crafts. For almost 5 years, the business was ok. With that business, he able to support his family, till one day, his shop was robbed. To make worse, he has no back up including insurance coverage. From that day, his life was miserable.

I received a SMS from by brother..

Brother: Bond..bole pinjam RM 1000.? Abang dapat business kontraktor buat jalan kat Kedah. Nak pakai modal. Nanti dah dapat duit, bagi balik la. Agak-agak boleh dapat duit sampai RM10, 000.

Me: ermm.....tak de duit la..belum gaji..ni belanja pun mintak kat ayah.. (B*bi sangat kan!!!!).

Brother: okay la..sorry kacau.

After 2 months, then we received a call from my sister in law. My brother has left home.
Apparently, my brother has borrowed a money from a loan shark. He just borrowed only RM 1000. He already paid the debt with an interest almost RM 5000. But, the loan shark still asking for more money as the interest balance was another RM 8, 000.

My mum asked me to help my brother. I have no money left. That RM 10, 000 was spent in just a month. I've no choice to make personal loan from local bank.
It was approved in a week and i'm able to pay the debt.

Despite that, my brother still missing. We had received a few SMS from unknown person asking where is my brother and asking for more money.

We lodged a police report early 2005 but till today he is no where to be found. We heard rumor he was in Bangkok, Thailand doing ordinary work.

I wish there is time machine where i can traveled back to the past and give my brother RM1000.

Feels much better today.
I already started working as usual.
Fever no more. Headache and cough much less. Still having sore throat. My voice still husky but my nurse said it's sexy!!!.

I would like to say Thank You for all the wishes and prayers.
Special thanks to Mr Iqmedia for nice dinner treat and the trip to Taman Jalil Alip.
I had the best crabs soup ever. I will published the photos during our trip to Taman Jalil Alip later.
