SNAPSHOT: 11th Sabah State Clinical Conference

Every year the Jabatan Kesihatan Negeri Sabah, organizing a conference called "The Sabah State Clinical Conference". This year the HDOK was given this task to organized the conference in Sandakan. After half a year of preparation, the conference kick off from 22nd to 24th October 2009. It was held at the Sandakan Nursing College. This year the theme selected was "East Meets West to Attain the Best in Pain Management Care".
This theme was selected as our hospital was been chosen to start a new service called the "Traditional and Complimentary Medicine (TCM)". This is a new concept, approved by the Ministry of Health, where the TCM will be incorporated into the modern medicine. As a starting point, we have chosen a chinese acupuncture medicine and malay massage. The Chinese acupuncture medicine will be dealing with patients with chronic pain and the malay massage will be dealing with patients after giving birth (urut lepas bersalin). If everything goes well, we will slowly introduced herbal medicine in our hospital.
This event was a success and everything went smoothly..

I'm really proud of myself (bukan riak yea) after being selected to present among the country top researchers.

Beyond Skin Deep







Get under the skin of what it is to be human.

The hugely popular exhibition "Body Worlds & The Cycle of Life" has opened at the Singapore Science Centre.

To achieve its striking purpose, the show has used nearly 200 real human specimens and about 20 whole bodies that have been 'plastinated'.

These are displayed in all sorts of poses to showcase what bodies look like within, through the phases of life from youth through to old age.

The exhibition will be on until March 6 2010.

Looking at those pictures are awesome. It just like my anatomy textbook displayed in real.

Okayy..lets do some saving and fly to Singapore.

Antara 2 Darjat



Antara 2 Darjat (Between 2 classes) is a 1960 Malaysian drama film directed by and starring Malaysian legend P. Ramlee. I was introduced by this film by my mom. She always cry watching this movie but i usually fall asleep mid way. This is a story about a love between a rich girl to very unfortunate poor boy. As usual, the rich father which has all the power always not happy in their relationship as the poor boy is not same "standard". But like others epic movie, their true love destined them to be together.

This is the synopsis of this film which i copied from wikepedia:

The film tells the tale of Tengku Zaleha (Saadiah), the daughter of a wealthy man, who is in love with a poor pianist named Ghazali (P. Ramlee). Tengku Zaleha is the only daughter of Tengku Karim (Ahmad Nisfu), a very class-conscious landlord who has two children, the other being Tengku Hassan. Hassan is from Karim's deceased first wife while Zaleha is from his second (present) wife, Yang Chik (Rahimah Alias), who is not of royal blood and is an ex-cabaret girl.

After Zaleha completes her studies, her father buys her a bungalow in Johor Baru where she meets and falls in love with Ghazali after he performs at her birthday party. Karim hears of the romance and decides to stop it. He gets his men to beat up Ghazali and at the same time takes his daughter back to Singapore.

Yang Chik resents Karim's way of treating Zaleha. She believes that Zaleha should be allowed to marry the man of her choice. However, Karim and Hassan insist that Zaleha must marry her cousin, Tengku Mukri (S. Kadarisman), who is of the "right" lineage. In a heated and violent argument that follows, Hassan hits Yang Chik with a heavy object and kills her. Hassan is arrested and subsequently sentenced to death. Karim is terribly upset with the death of his wife and only son and becomes mentally unstable and ends up in an asylum.

Mukri's father, Tengku Ismail, confronts Zaleha with a letter which he claims was written by Karim. After reading it and believing it to be genuine, Zaleha has no choice but marry Mukri. After the marriage, they leave for Zaleha's bungalow in Johor to spend their honeymoon. They are accompanied by a close friend, Tengku Aziz (Yusof Latiff).

When the piano at the bungalow is out of order, a tuner is called to repair it. Zaleha is both shocked and happy that the tuner is Ghazali, her former sweetheart. After repairing the piano, Tengku Zaleha played the piano with the song that Ghazali taught her before. Ghazali is shocked and ran away from the house.

At his home,Ghazali told her mother and his cousin,Sudin(S. Shamsuddin) that Tengku Zaleha was still alive. But Sudin then claimed that Ghazali is mad. Soon,he is beaten by Ghazali.

Later that night,Tengku Zaleha met Ghazali and told him what happened to her. Ghazali is happy because his sweetheart was still alive.

The next day,Tengku Aziz told Tengku Mukhri to divorce her wife and tell the truth about Tengku Zaleha. Tengku Mukhri was mad when his friend told him about the affair between Ghazali and Tengku Zaleha. He then sent Tengku Aziz to Singapore and on the way, he "kill" Tengku Aziz.

Back to Anggerik Villa,he took his gun to hunt Ghazali. Soon,he arrived to Ghazali's home and fight with Ghazali. He almost kill Ghazali,but then Tengku Aziz kill him with a pistol.

Ok, now lets read my story...

Case 1.

A 40 year old lady came to see me at the surgical clinic a year ago. She is an immigrant, from Mindanao, Philippines. She had migrated from her country about 10 years ago and she is working as a laborer. She has 5 children. Her husband was no where to be found.

She complained of painless lump over her right breast. It was small. As usual i did an ultrasonography of the breast and a biopsy. The biopsy was reported as breast cancer.

From the 1st consultation to the diagnosis she had paid almost rm 220 .This is the breakdown of her payments:

1st consultation: rm 60

Breast ultrasound and biopsy: rm 100

2nd consultation: rm 60

All non-citizen patients was charged as private patients as the government has make a rule that the non-citizens are not allowed to enjoy the "SMS"..

I break the news saying that she got cancer. She seems understood and reply that she want to get better and will follow what ever i said.

I told her she needs surgery and later chemotherapy. BUT as a non-citizen, before the surgery she must pay first ie about rm 600. And each time before the chemotherapy she has to pay rm 100/cycle..

She started to cry and repeatedly saying that she has got no money...

I replied back and asked her to go back to Philippines and seek treatment there..

Again, she replied back "in Philippines it's even more expensive..and poor people like us can't get access to the health care"..

She left the room and i never see her again.

Case 2.

A 40 year old lady came to see me at the surgical clinic a year ago. She was accompanied by her husband. She is a local Sandakan. She is married with 2 children and works as a government officer.

She complained of right breast lump for a year. The breast lump was HUGE, it occupying almost the whole breast. Besides that she also had cough and a back pain.

As usual i did an ultrasound of the breast and a biopsy. As expected it is a cancer. And the xrays showed that the cancer already spread to the lung and bone (pelvis).

From the 1st consultation to the diagnosis, she didn't paid a single cent. Everything is FREE. As a Malaysian citizen and a government officer, she is entitled for the "SMS" .

I told her the diagnosis. Despite the cancer already spread, she appeared relax.

After a lengthy explanation i advised her to go for chemotherapy. The chemotherapy is expensive as this is a new drug that has just discovered. A single bottle cost about rm 1500, and she needs at least 2 bottles per cycle for 6 cycles!!

Again,,,this is provided FREE.

She refused the chemotherapy.

She left the room and i never see her again.

ONCOLOGY CLINIC

Dr D Jayendran.

Assalamualaikum dan salam sejahtera.

Entry kali ini ditujukan khas kepada Dr D. Jayendran.
Dr D Jayendran adalah pakar onkologi negeri Sabah. Onkologi adalah mengenai perawatan penyakit kanser.
Perawatan Kanser di Sabah bukanlah baru. Sebelum ini, terdapat pakar onkologi dari Kuala Lumpur yang membuat lawatan ke Queen Elizabeth Hospital, Kota Kinabalu sebulan sekali.
Cuma dengan kehadiran Dr D Jayendran bermula 2006 telah membuat perubahan besar dalam perawatan kanser di seluruh negeri Sabah.
Beliau berasal dari Klang, Selangor. Sebelum berpindah ke Sabah, beliau merupakan pakar onkologi negeri Pulau Pinang.
Semenjak beliau menjadi pakar onkologi negeri Sabah, banyak perubahan yang beliau lakukan..seperti menubuhkan jabatan onkologi dan radioterapi di Hospital Pakar Likas.
Tetapi, yang paling kami hargai adalah perkhidmatan "link" servis onkologi dari Likas, Kota Kinabalu ke hospital-hospital daerah Tawau, Keningau, Sandakan dan Lahad Datu.
Di Hospital sandakan, DrBond adalah pegawai perubatan "link" yang telah diamanahkan untuk menjaga pesakit-pesakit kanser di daerah Sandakan.
Perkhidmatan "link" disini bermaksud rawatan onkologi dari Likas dipindahkan ke Sandakan melalui pegawai perubatan "link" setelah mendapat persetujuan dari Dr D Jayendran.
Selama 4 tahun juga, tanpa tertinggal satu kalipun. Dr D Jayendran telah membuat lawatan bulanan ke klinik di Sandakan . Adalah menjadi tugas DrBond memastikan klinik ini berjalan lancar dan semua pesakit mendapat rawatan sempurna. Ini termasuk juga memastikan pesakit dihantar ke KK untuk rawatan susulan.
Memang penat menjaga klinik onkologi ini. Selain dari tugasan lain di Jabatan Pembedahan, tugas sampingan ini penuh dengan cubaan. Sila lihat entry -entry sebelum ini.
Hari ini kami juga mempunyai Klinik Onkologi.
Cuma berbezanya kali ini...ini adalah Klinik Onkologi yang terakhir. Dr D Jayendran telah membuat keputusan untuk bersara awal dan join private practice. Sehingga entry ini ditulis, tidak ada pakar pengganti.
Ini bermakna, klinik onkologi di sandakan yang baru nak subur...akan terkubur.

SMS dan "the father of all chicken"

the father of all chicken...
SMS bermaksud sudah mau subsidi..

Cerita 1...
Masa Dr Bond kecik-kecik, Dr Bond berpeluang membela ayam bersama sama ayah Dr Bond..
Tak la banyak mana, cukup untuk jual di pasar cina sesekali..dan bole la buat lauk ayam seminggu sekali.
Dalam banyak-banyak ayam, ayah Dr Bond cukup sayang seekor ayam ni..ayam jantan,,nampak lawa dan gagah (dimata ayam-ayam betina la)..tapi dah tua dah.
Kami panggil dia..'the father of all chicken' atau bapak ayam.
Dia berjaya mendapat gelaran itu kerana dia berjaya membuntingkan (is this a correct word???) mak-mak ayam kami...kami pun dapat la telur-telur ayam dan seterusnya anak-anak ayam. Anak -anak ayam tu pun membesar dan kami pun dapat hasilnya.
Sungguh berjasa 'bapak ayam' kepada kami sekeluarga.

atu hari mak Dr Bond cadangkan sembelih saja ayam tu...lagipun dah tua..sekurang-kurangnya bole la buat sup ayam.
Ayah Dr Bond dengan marah berkata:

Ehh!!! Tak bole la,,,ayam ni dah banyak berjasa dengan kita. Aku tak sanggup nak sembelih.

Mak Dr Bond pun jawab balik: ok la ..ok la...tak nak sembelih, lepas kan la ayam tu dari reban,,buat apa kurung lagi.

Kemudian, kami pun sepakat la melepaskan ayam itu dari reban.

Selepas 2 hari diluar kurungan, Dr Bond nampak ayam tu kurus dan sakit.

Dr Bond tanya abang DrBond kenapa "bapak ayam" jadi macam tu..Abang Dr Bond cakap dia mau SMS la...Kemudian dengan panjang lebar abang Dr Bond menerangkan apakah itu SMS..

"Bapak ayam ni dari kecik kita jaga dalam reben..Sejak dia menetas kita bagi dia makan ..
sekarang bila dia dah keluar dari reban...dia tak tau nak cari makan sendiri...tu la SMS...suda mau subsidi...

Akhirnya..bapak ayam tu mati 2 bulan kemudian (Bukan mati sebab tak makan..tapi sebab dah kena makan!!!!)...


Cerita 2....

A 35 year old lady came to see me in surgical clinic about a year ago.
She is local from Sandakan, and she is unemployed. She is staying with her sister who works in the hospital as a hospital attendant.
She complained of right breast lump for a year.
My clinical impression was highly suggestive of breast cancer.
She underwent ultrasound of the breast and a biopsy of the lump.
The biopsy came back..and its was confirmed as cancer.
From the first clinic consultation and to a diagnosis of breast cancer, she just paid RM 15..
As a comparison, in the bracket is the usual fees from private clinic/hospitals..
1st consultation: RM 5 (private clinic: rm 50-60)
Ultrasound breast and biopsy: RM 5 (private clinic: rm200-rm 300)
Second consultation: RM 5 (private clinic: rm 50-60)

Subsequently she underwent right simple mastectomy and axillary clearance...this is a surgery for the patients having breast cancer.

It costs about RM 100 including 3 days of hospitalization..(private hospital eg Sabah Medical Centre, it costs about RM 4000-5000)..

After that she underwent chemotherapy...to kill any remaining cancer cells. She had 6 cycles of chemotherapy, each cycle of chemotherapy she has to pay about rm 50 (in private hospital, approx rm 200/cycle).

After completion of the chemotherapy, then she needs radiotherapy.
In the whole state of Sabah, the government hospital do not have a radiotherapy machine. Any patients require a radiotherapy, we will have to be refer this patient to Sabah Medical Centre.
It costs about RM 4000 for a duration of treatment of 6 weeks...but patients from any government hospital is free.
This is what we called as "buying service"...the private hospital provide the service and the government will pay all the cost.

This radiotherapy have to be done in Kota Kinabalu...and i have to send this patient to KK..

Everything has arranged...and appointment already taken..

Then a week before the scheduled appointment the sister came to see me..

The sister: DrBond....nanti adik saya nak pegi KK minggu depan...bole tak refer ke kebajikan..

DrBond: refer kebajikan??? kenapa??
The sister: nak minta tiket kapal terbang...

DrBond: nak naik kapal??? dia ok la...naik bas pun boleh. Kami reserved untuk naik kapal terbang untuk kes kes pesakit yang tak boleh jalan...

The sister: ala Dr Bond bole la...my sister tak bole jalan jauh..nanti dia pening dan muntah2..

DrBond: tak bole..NO!!

Then the sister with a sour face slammed my clinic door while getting out from the room.

About 5 mins later i called her back (i just don't want to receive any complaints)

Ok lah...saya refer kebajikan...tetapi bergantung kepada kelulusan Puan Pengarah!!!

The next day, as expected i received a call from Puan Pengarah..

Puan Pengarah: Bond...why you want to send this patient by air.?????.
DrBond: (pening!!!)

The sister had reminded me of "bapak ayam".

Soalan: LITERAL THINKING.

HAPPY DIWALI

May the festival of Lights, Diwali dispel darkness, ignorance and evil from the world.

I'M SICK

Not feeling very well today. I had URTI, upper respiratory tract infection: fever, sore throat and cough.
Despite that, managed to go work today. I'm just lucky, today is not busy. After the ward work, i just slept in side the oncall room and continue working in the afternoon.
Each time i got sick and URTI, i'll make sure i had these:

1. KFC threat.
I went to KFC restaurant in town, near the Sandakan Harbour. My taste buds are not working when i'm sick, most of the food is sour and i had no appetite,,, except KFC.. Don't know why, it could be the 11 secret herbs that give the flavor.

It's finger licking good..
Waiting for my turn.
Yummy

2. Drink plenty of water.
I went to Genting Mall mall, just next to the KFC. Bought 3 bottles of carbonated drink, to prevent from dehydration,,,as fever can cause fluid loss and eventually dehydration.


3. Medication.
I'm self prescribed with 3 medications:
a) Panadol: to control the fever.
b) Cough mixture: to reduce cough
c) Vit C: to improve my immune system.
My medications..
I have problem in swallowing medication tablet especially Panadol. The sweet carbonated drink help me to swallow it.


4. Handphone...needs to call my bos and tell him that i'm sick and i can't do oncall tonight.

5. Take a shower.

6. Eats some more..

7. and Sleeps...

Hopefully, by tomorrow morning i'm recovered.

MOS Meeting 2009



MOS stand for : Malaysian Oncological Society. This year the Annual Scientific Meeting of MOS will be held from 5th to 8th November 2009, at Johor Bharu. Ive submitted an abstract for the meeting and i just received an email today saying that the abstract was accepted.

Dear Dr James Bond Malaya,

I am glad to inform you that the Scientific Committee of the MOS Annual Scientific Meeting The Southern Chapter 2009, has accepted your paper on "Breast Lump Marking Using U/sound and Methylene Blue Injection in Early Breast Cancer Treated with Breast Conservation Surgery" to be presented during this meeting.

The scheduled presentation will be on Saturday, 7th November from 3.30 pm onwards at hotel The Zone, Johor Bahru. You will be informed later in which room you will be presenting.

You will be given between 10-15 minutes to present and if time permits, a short Q&A session will follow. A panel of judges will be hearing to your presentation and the best three papers will be given an award.

Please CONFIRM with us if you agree to present within this week, to enable us prepare the final list of presenters. As a gentle reminder, the MOS will be not able to undertake any expenses incurred upon you to present at this meeting.

Friday Prayer



Attended Friday Prayer today..

Actually i want to escape the prayer,,and straight away to the "The Mall", a shopping complex near putra world trade centre. Upon reaching at the commuter station in Putra, then i heard an azan from the nearby mosque. Just like a magnet, i was pulled inside the mosque.

What a surprise, this is not an exactly a mosque. It is an Indonesian Embassy in Kuala Lumpur.

Another surprise, the prayer hall actually is the covered basketball hall.

They just put a layer of sejadah and a mat. No aircond or nice carpets like our own mosque.

The friday prayer started with an anouncement to appeal for the donation for the earthquake victims in Sumatera.

Then, the lecture start as usual. It was given from a khatib, with a strong Indonesian dialect.

Today the main topic was about "Persaudaraan among the Muslims".


This is the summary:

This is you should do to establish a good persaudaraan among the Muslims.

1. You must give salam each time we meet.

2. You must 'bertanya khabar"...like sending sms asking how are you doing..or just simply a visit.

3. You must visit and take care of them when they are sick.

4. You must also escort them to the graveyard when he dies.

5. (i forgot).


The lecture is simple but really practical...and i think we should do the same to our nonmuslims members.

In KL for exam..the BSSE: Basic Surgical Science Examination..

Went to University Malaya Medical Centre at 7am where the exam is going to be held..
The exam (part 1) started at 8 am and finished at 12 mid day..
this was please..
There were 53 candidates registered for the exam,,each was given a registration number. Mine was 007.

10 candidates didn't turn up for exam (don't know why),,left only 43 candidates.

The result released at 2 pm the same day. Out of 43 candidates, only 17 candidates passed the part 1 exam and were allowed to continue the exam (part 2) the next day..
The no 007 was not on the list..

Trip to Kuala Lumpur

This is my first trip report on any airplane. This report is exclusively for a trip to KL on 7th October 2009. Any feedbacks are greatly appreciated.

Background and introduction.
SDK to KUL is one of my routine traveled route. Away from home in KUL make me have to travel his route about once in 4 months, sometimes even more.MAS and its main rival AIRASIA are the only carriers serving this route. Before this MAS had only 1 flight from KUL to SDK (vice versa), with a transit at Kota Kinabalu. Actually MAS had introduced a direct flight in the year 2005, but it was stopped because of poor yields as compared to AIRASIA.
But this is now a history, starting Mac this year MAS has reintroduced a direct flight with a competitive airfares (its really cheap,,sometimes cheaper than the low cost carrier).
And to a surprise, MAS added second direct flight starting last month. (Off course,, cheaper airfares with a 5 star airlines,, who will refused to grab the opportunity).

The beginning..
I wake up late this morning,,at 6 am. Its not the alarm bell that woke me up ...but its the nature call. I had a loose motion and "sakit perut"...it's must be the chicken curry i ate last night.
Ohh this is not good as I've to travel to KL today.
Settling that i continue sleeping and woke up at 8.30 am, and immediately rush to the bus stand.

The journey...
Hoping for a saving, i waited for a bus to the airport. None has arrived. It's almost 9.30am, and i had another 1 hour for check in.
Then, without much thinking (i have to) , i took a taxi to the airport.
What a surprise..the taxi driver is my nurse husband.
Exactly, at 9.45am, i was at the airport.
Check in counter was full.. its going to be a full flight today.
I was greeted by a malay lady. She looks nice...with the MAS kebaya and a green tudong..
Check in was smooth.

Alright,,this is your boarding pass..and seat 27F,,just near the toilet..

Each time i traveled, i always asked a last row at the back of the aircraft...Don't asked me why..it's just my personal preferences.

Looking at this airport was awesome..it simple but looking the roof top was nice..I don't think there are others airport in Malaysia like this.

Straight away, i went for immigration clearance. The immigration officer was a malay lady with a white tudong. I gave a salam,,and handed over the document.

Going to KL.????..
Yes,,i replied.

She handed over back the document and i went to boarding hall. It was full and there was MAS flight to Kota Kinabalu. It was delayed, as its almost 10.30am and the passengers have not boarded the flight yet.

I haven't had any breakfast and i went to the cafe. Nothing much food to choose and i just had a cup of coffee. Then, someone called my name...

Bond, what are you doing here???
I'm going to KL lah,,got exam tomorrow..

I went to climb mount kinabalu a few days ago. managed to climb to a top..very tired la..just arrived at sandakan, last night...you should go climbing la BOND..

After nice small chat.i headed to my seat. Then another surprise..i met a MAS steward ..who was my ex house mate during uni time. I haven't met him almost 10 years..
He showed me my seat.
As usual, the chief cabin crew made welcome announcement and follow with the usual routine safety demo.

After an hour, the inflight services started. The good thing of sitting at the last row was, i was first to be served.
Some where over ?Kinabatangan..
There were 2 choices for lunch: a chicken beriani or fried rice with sambal udang. I choose the later.
Meal..
It was served hot and delicious. Soon the usual drinks service starts: an offer of hot drinks or juices..
After had the meal, i went to the galley and met my ex-housemate. He introduced me to his colleague, Miss Annabelle. We just chatting about what we are doing right now..He has been with MAS almost 5 years.
My ex-house mate, an accountancy dropout student, joined MAS five years ago.

The 2 hours and 45 mins journey was smooth, except some minor turbulence over south china sea.

At 1.50pm the captain made an announcement that we were descending. We landed safely at 2.10pm.
